We assign an experienced and
qualified Case Manager Nurse to you, to ensure that all
of your family’s needs are provided for and coordinated.
All clients and staff know that they can call any time,
day or night, to speak directly to a nurse. When Best
Home Care places a paraprofessional in your home and a
service or clinical problem arises, a nurse is available
at all times to troubleshoot and resolve issues.
Our nurses also provide medication management,
monitoring of vital signs and health status, wound care,
IV therapy and care for hospice clients and their
families. We notify physicians when there is a change in
client status to ensure that the client receives
appropriate medical management. When needed, we
accompany clients to medical appointments so that
medical issues are addressed and that the family
understands what has occurred during the visit as well
as treatment options and care plans.
